New Patient Registration

We are welcoming new patients! 

We will need you to fill in a few forms: GMS1 Registration form, New Patient Questionnaire and Record Sharing.  You can collect these from reception in the registration pack or you can download the forms below:

We will also want to see the following documents:

  • Government issued Photo ID document (e.g. driving licence or Passport)

In order to register as a patient you will need:

  1. Complete GMS1 (purple form)
  2. New patient questionnaire for adults/children under 16 years/new born baby. (please see seperate links for these depending on age/s)
  3. You need to complete these forms fully for us to register you. Incomplete forms will be rejected. 
  4. We require complete immunsation records for all children under the age of 16 years.
  5. We recommend that you sign up to online access as it allows you to access your medical records, view your test results, manage your appointments and much more, using the NHS App or SystmOnline App.

The surgery catchment area is very wide and is demonstrated in the image below. We will offer registration to everyone with 

in the blue area however only people within the Red highlighted area will be eligible for a home visit.
